One of the most exciting advancements in veterinary dermatology is the use of laser technology. We are very proud to be one of only two veterinary dermatologists in England and Wales to own the Vetscalpel CO₂ laser.
This machine has been updated over the years and now offers even more power, resulting in fast and safe excision or ablation of a wide variety of skin lesions where other modalities are either extremely difficult and bloody or even impossible.
A CO₂ laser enables the effective removal of tissue with minimal thermal damage to the surrounding tissue, whether used for excision or ablation. Through Haemostasis the nerve endings are sealed, which results in far less post-operative pain and swelling compared to conventional surgery. This level of precision is particularly valuable when working within the delicate structures of the ear.
One of the main reasons we invested in the Vetscalpel laser was to be able to remove ear tumours effectively and save the animal from having to undergo a much more expensive Total Ear Canal Ablation operation.
Ear tumours are a primary cause of otitis externa because they occlude the ear canal, trapping debris and encouraging infection. Using laser surgery, we can often remove these growths cleanly and efficiently, restoring airflow and allowing the ear to heal naturally, with less pain.
When Dog Ear Surgery Becomes Essential
Despite advances in medical and laser treatments, there are cases where more extensive dog ear surgery is necessary. Chronic or end-stage ear disease can lead to irreversible changes within the ear canal, making long-term revolution through medication alone impossible.
In these situations, we may recommend a Total Ear Canal Ablation combined with a Lateral Bulla Osteotomy (TECA-LBO). While this is a more complex procedure, it is often the most effective way to eliminate pain and restore quality of life.
Indications for TECA-LBO Surgery
This form of dog ear surgery is typically considered in cases such as chronic end-stage otitis, where there are irreversible changes to the ear canal that prevent resolution of pain, discomfort and infection. It is also indicated in cases of intractable otitis, where deeper structures such as the middle ear or tympanic membrane prevent full healing despite treatment.
Tumours within the ear canal that cannot be addressed through less invasive procedures may also require this approach. In some situations, the overall cost of surgery can even be less than prolonged medical management, which may involve repeated anaesthesia and procedures without a guaranteed outcome.
The Complexity of TECA-LBO Procedures
As a dog ear specialist, we emphasise that TECA-LBO surgery is not routine and should only be performed by an experienced surgeon. The procedure involves removing the vertical and horizontal canal, the integument attached to the bone at the opening of the middle ear, the ear drum and part of the lateral wall of the middle ear. The middle ear is then thoroughly cleaned, and any diseased tissue is removed.
